When you come in for the first time, you can expect a thorough exam followed by a discussion of our findings and treatment recommendations.

Before your first visit, we will mail you a health history form. Please bring the following information with you to help make your visit smooth and efficient as possible:

  • Your referral slip and x-rays from your dentist (you may request your dentist to forward these information to our office)

  • Completed health history form with a list of medications you are currently taking
    (Please click HERE for a copy of the health history form)

  • Dental insurance card (if applicable)


Important: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.
